As a client, you have the right to:
1. Be treated with courtesy, dignity, and respect regardless of age, race, religion, gender, disability, or background.
2. Be free from abuse, neglect, exploitation, harassment, or discrimination.
3. Receive services in a safe environment.
4. Participate in decisions regarding your care and services.
5. Refuse services or request changes to services without fear of retaliation.
6. Have your privacy respected, including confidentiality of personal and medical information.
7. Voice complaints or concerns without fear of retaliation.
8. Be informed about how to report abuse, neglect, or grievances.
9. Access your service plan and request clarification at any time.
10. Be informed of service costs and billing practices.
11. Receive services from qualified, trained staff.
12. Have your cultural values, personal preferences, and independence respected.
